Supervises all clinical operations within the Cantata At Home program, including supervision of employees to ensure compliance with all quality standards, in ac1. Supervises, develops, motivates and evaluates clinicians and healthcare workers.
2. Makes regularly scheduled visits to client homes to ensure that the caregiving staff
is providing care as directed by the Registered Nurse.
3. Supervises assigned clinical operations and ensures compliance with federal, state, local laws, and Cantata policies, procedures, and standards.
4. Maintains a work environment conducive to achieving professional goals among the employees, i.e. training, educational resources, etc.
5. Participates in interviewing, hiring, separations; and provides orientation and training to employees.
6. Identifies performance issues, discusses with the Manager and assists in their resolution.
7. Manages the operations of the assigned unit, including achieving the business plan.
8. Promotes the growth of the business by presenting information, education, and support regarding Cantata Services.
9. Assists with the implementation of new company-sponsored systems.
10. Collaborates with the Manager to create and/or implement employee recognition, reward, and incentive programs.
11. Analyzes and evaluates assigned clinical operations to ensure standards are adhered to.
12. Completes other assignments as requested and assigned.
13. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.

Job Evaluation Criteria
Home Care Supervisosr
A. Knowledge/Skill/Experience
Ø Registered Nurse currently licensed in the state of Illinois
Ø (Per Diem Position: licensed LPN, Social Worker, or Counselor).
Ø Three (3) years of current nursing experience.
Ø One (1) year of experience as a nursing supervisor
Ø Demonstrated excellent supervisor skills
Ø Must possess leadership ability and willingness to work harmoniously with and to supervise other personnel.
Ø Ability and willingness to effectively motivate and lead individuals and groups with diverse needs and abilities.
Ø Must possess the ability to make independent decisions when circumstances warrant such action.
Ø Must be able to maintain good personnel relations and employee moral.
Ø Must be able to cope with the mental and emotional stress of the position.
Ø Ability and willingness to work harmoniously within a team environment departmentally.
Ø Ability to communicate effectively both verbally and in written form with a diverse group of people.
Ø Must possess the ability to deal tactfully with personnel, residents/patients, family members, visitors, government agencies/personnel and the general public.
Ø Must be able to read, write, speak, and understand the English language.
Ø Must have a valid driver's license

B. Contact with Others
Extensive contact with residents/clients on a daily basis. Daily contact with all levels of personnel within the company. Heavy interaction with resident's/client's family members and community contacts.
C. Delegation of Authority
As Home Care Supervisor, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
D. Responsibility for Accuracy
Omission or incorrect mandatory federal, state and facility documentation can generate penalties enforced by regulators.
E. Responsibility for Confidential Data
Employees are required to safeguard sensitive information. Sensitive information includes, but shall not be limited to, information concerning Cantata's former and current residents/clients, medical and health information and Cantata's information systems.
F. Physical Requirements/Working Conditions
Ø Works in residents' rooms and/or clients' homes, as well as in a typical interior/office work environment.
Ø Must meet general health requirements as set forth by this facility.
Ø Intermittent physical activity including bending, reaching and ability to frequently life and carry up to 50 pounds in order to perform daily job functions and related activities that may be required. Frequent walking, standing, or sitting most of the time. Using arms or legs to push/pull.
Ø Sits, stands, bends, lifts, walks and climbs stairs, as needed.
Ø Individual will be scheduled based on operational need.
Ø Individual will be required to work at client's homes and be responsible for own transportation.
Ø May be requested to work over scheduled working hours, weekends and holidays when necessary.
Ø May be necessary to assist in the evacuation of residents during emergency situations.
G. Exposure Classification
Ø Job duties are classified as Category I (routine exposure to blood, body fluids or tissues).
Ø Routine exposure to chemicals that are identified for use in the nursing department.
