FAST GROWING Home Health Agency (Kelly Home Healthcare Services) Require for immidiate hire, A PART TIME RN CLINICAL CARE COORDINATOR/QA (3 DAYS A WEEK)
This position is required for our Flossmoor IL Office location.

This position will coordinate home healthcare services provided to patients in which nursing is the primary discipline. Review the initial assessment, develops and implements the plan of care in coordination with the physician, coordinates care with other disciplines, performs discharge planning. Works with all team members and office staff as a patient advocate in obtaining health-related goals.

Ensures adequate service provision, including the scheduling of visits, based on the patient's identified needs and priorities for care.
Electronically manage patient's health Record, develop Plan of Care, Transmit Oasis when necessary and ensure adequate receipt and review of all patients' progress report and Nurses Visit Notes
Advice the patient or family in self-care, assesses and monitors clinical status, administers medications, draws labs. (When necessary)
Has good understanding of Medicare regulations and eligibility requirements for home healthcare and follows all regulatory requirements. Care is directed towards teaching the patient/significant other how to manage the illness at home, assist the patient through the rehabilitative process, and teach them how to return to the highest level of function possible to maintain independence in their own environment.
You will assist the DON with the Quality Assurance of patient care records and accurately coordinate admissions.
Full knowledge of OASIS and 485
Coordinating with therapists to discuss patient cases

Also, you must have an excellent work ethic, good people management skills and the ability to perform well in stressful situations. Additional requirements include:

This position requires a Self-directed RN, who is able to work without direct supervision. Possesses excellent coordination, multi-tasking, organizational, and communication skills.

Qualifications

1. Minimum of two (2) years of Medicare Certified Home Health and or Hospice nursing experience.
2. Prefer at least 1 year of clinical experience as a full time Registered Nurse with skills in cardiac, measure or critical care nursing as well as experience with the adult or geriatric population. Prefer knowledge of homecare requirements and OASIS requirements and competency.
4. Must have experience in chart Audit and must have used Visitrack or Axxess software.
5. Must have Active and current Registered Nurse license with the State of Illinois. "
