General Summary:
Responsible for the medically related provision of social services to the residents and families of Cantata. This includes responsibility for the emotional, social and behavioral needs of the residents. Each resident's needs are to be addressed as indicated by their individual circumstance and condition--medically and socially. Collaborate with Transitions Care Manager; Transitions Marketing Specialist and Interdisciplinary team in customizing transition plan that will address the client's needs base on a comprehensive assessment. Following Federal and State standards along with Cantata's policies and procedures, which govern our facility is the format for the provision of these services also to include independent and professional judgment.
Principal Duties and Responsibilities: This description is intended to identify the basic

1. Develops a comprehensive social history and psychosocial assessment that includes the resident's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
2. Reviews Advance Directive at the time of admission and assist in completion as needed and review annually.
3. Helps residents and their families cope with the immediate effects of the decision to move to a facility permanently or for short-term sub-acute rehabilitation to enhance customer satisfaction.
4. Orients residents and families to the facility, its services, and its service limitations within the Health Care Center. Collaborate with Transition Marketing Specialist on living options and services on campus.
5. Notifies, in writing, and schedules care plan meetings with residents and families encouraging their attendance at care conference.
6. Attends scheduled care conferences and identifies and interprets psychosocial needs of residents for inclusion in the care plan, addressing in particular issues of schedule, treatment, and environment that express and reinforce individuality and identity.
7. Completes mandatory (RAI) documentation within timelines established by applicable regulations.
8. Empowers residents and encourages resident choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices based on racial, cultural, and ethnic heritage and sexual orientation.
9. Charting as required, incidental and interventions.
10. Collaborate and refer to Transition Care Manager to Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment etc.)
11. Collaborate and refer to Transition Care Manager and/or Transition Marketing Specialist to Assists residents and families with financial questions and makes appropriate referrals.
12. Develop and involves families in family informational/support meetings monthly or quarterly. Host support meeting for Children of Adult Parents at Cantata in collaboration with The Customer Relationship Team.
13. Helps residents and family prepare for and cope with losses, including aging and death. Reviews or provides Advance Directive Information as needed. Manage; implement and care plan all hospice cases. Collaborate with Transition Care Manager on Hospice client's going home.
14. Collaborate and refer to Transition Care Manager for developing a comprehensive customized transition plan by speaking with client, family, multidisciplinary team, and physician. Transition Care Manager will implement and coordinate outside resources, equipment and medication needs, which identify the client's needs and preferences.
15. Maintains resident room change request list, informs resident when changes are made with their room-mate
16. Involves the entire facility in meeting resident's psychosocial needs.
17. Trains and consults with staff regarding psychosocial needs that encompass social, ethnic, religious, and cultural diversity of individual residents, the resident group as a whole, and families to increase staff sensitivity.
18. Educates staff about the Resident Bill of Rights and their responsibility for supporting such rights.
19. Participates in policy decision making in a way that encompasses an understanding of the issues and implications relevant to religious, ethnic, and cultural diversity as it affects resident life and care and family involvement.
20. To be an active part of a The Customer Relationships team that will foster and maintain relationships that will Attract, Retain and Serve all current and potential future clients, hospitals, community agencies, and other referral sources.
21. Support and assist as needed, with general marketing initiatives that will enhance customer satisfaction as determined by the Administrator and/or the Customer Relationship Team.
22. Performs administrative functions such as completing necessary forms and reports.
23. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
24. Attend in-services, meetings and educational training in accordance with The British Home's policies and procedures.
25. Consistently demonstrates ability to respond to changing situations in a flexible manner.
26. Perform tasks/duties and utilize forms in accordance with Cantata's policies and procedures.

Social Services Designee
Knowledge/Skill/Experience
Ø Bachelors Degree in Social Work from an accredited School of social work.
Ø Must have a minimum of one (2) years supervised social work experience in a health care setting working directly with individuals.
Ø Must possess a working knowledge of Illinois Department of Health regulations and OBRA.
Ø Prior experience with the admission/discharge process within a healthcare/rehab setting helpful.
Ø Must possess the ability to make independent decisions when circumstances warrant such action.
Ø Must be able to cope with the mental and emotional stress of the position.
Ø Ability and willingness to work harmoniously within a team environment.
Ø Ability to communicate effectively both verbally and in written form with a diverse group of people both inside and outside the organization.
Ø Must possess the ability to deal tactfully with personnel, residents/patients, family members, visitors, government agencies/personnel and the general public.
Ø Must be able to read, write, speak and understand the English language.
Ø Aptitude for detail work.
Ø Basic computer literacy.
Ø Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
A. Contact with Others
Frequent contact with residents on a daily basis. Daily contact with all levels of personnel within the company. Frequent interaction with residents, resident's family members, and community contacts.
B. Delegation of Authority
As Social Worker you are delegated the authority, responsibility, and accountability necessary for carrying out your assigned duties.
C. Responsibility for Accuracy
Omission or incorrect data may lead to a decrease in productivity departmentally. It may also lead to penalties enforced by regulators.
D. Responsibility for Confidential Data
Employees are required to safeguard sensitive information. Sensitive information includes, but shall not be limited to, personnel files, information concerning Cantata's former and current residents/customers, medical and health information and Cantata's information systems including but not limited to the Internet and E-mail.
E. Physical Requirements/Working Conditions
Ø Must meet general health requirements as set forth by this facility.
Ø Sits, stands, bends, walks, and climbs stairs, as needed to conduct social work activities.
Ø May be requested to work weekends and holidays when necessary.
Ø May be necessary to assist in the evacuation of patients/residents during emergency situations.
F. Exposure Classification
Ø Job duties are classified as Category II (no routine contact with blood, body fluids or tissues).
Ø No exposure to hazardous chemicals.
